The Coolpeel© protocol presents an innovative option for non-invasive photorejuvenation, utilizing a microablative fractional CO2 laser that minimizes downtime for patients seeking skin enhancement.
Technology

This treatment primarily operates in CO2 laser mode, characterized by a wavelength of 10600nm. Its uniqueness stems from two key technological features:
- High Pulse (HP) mode: This generates extremely short laser pulses with high peak power.
- Scanning mode in “spray”: This method distributes impacts randomly across the treatment area, preventing overlapping and creating small epidermal pits, referred to as DOTS.
The procedure results in pits that range from 70 to 120 microns in width and 32 microns in depth, with a coagulation depth of 80 microns. This technique is suitable for all skin types. Practitioners can choose between two protocols if they have access to the dual-wavelength version, CO2+1540 (Duoglide©). The CO2-only version focuses on the epidermis, while the combined version stimulates deeper skin layers, enhancing the treatment's overall effectiveness, especially in younger skin. Treatment of the full face can be completed in 10 to 15 minutes, while the neck or décolleté can be treated in about 10 minutes.

The Coolpeel© protocol is primarily indicated for:
- Skin photorejuvenation
- Improvement of complexion
- Enhancing skin radiance and glow
Although the protocol is tailored for facial treatments, it can also be applied to the neck and décolleté.
Treatment Options

Patients can select from several treatment options, tailored to their specific needs:
- Coolpeel© Light: Utilizes a low intensity CO2 laser setting (0.8-1 W) with a standard spacing of 900 microns between each pulse.
- Coolpeel© Classic: Increases CO2 laser power to 2 W, suitable for standard treatments, and can be combined with the 1540 Laser for additional revitalization.
- Coolpeel© High: Further escalates power to 3 or 4 W, with reduced spacing of 800 to 700 microns, enhancing subcutaneous matrix stimulation through the 1540 Laser.
Coolpeel© Compared to Traditional Peels
The Coolpeel© Light version can be likened to a rejuvenating peel, offering immediate but temporary radiance. The Classic version, operating at 2 W, parallels the effects of light peels such as 12% or 15% TCA peels, particularly in improving skin quality, texture, and reducing fine lines. When employing higher parameters with the 1540 Laser, the Coolpeel© can achieve results comparable to multi-layered 15% TCA peels.
This protocol serves as a complementary addition to skincare regimens and fits well within a diverse aesthetic medicine practice. However, it may not be suitable for individuals over 65 or those with specific skin conditions, such as “Glogau 3,” due to the potential for skin damage and the depth of wrinkles.
